What Will I need?
All patients are required to complete and sign a screening questionnaire prior to entering the magnet room. This form is necessary for your safety and protection. Some surgical implants are not safe to enter the magnetic field; therefore, we ask for your cooperation completing this form as accurately as possible.
• A locker is provided before the exam for all valuables. • Please bring your driver’s license or state ID and insurance cards with you.
